In the Merchant Administrator, go to the International Settings. Click on Clear (Erase) Country List. Then click on Edit Country List. Click on Tuvalu - Zimbabwe, then find the field next to United States of America [U,P]. In the field type United States of America, or whatever your want to appear in the drop-down box. Depending on what services you use and what countries you ship to, you may want to configure the country list differently for UPS or USPS, which you can do by checking the appropriate box and clicking Set Country List Based on Service.

Note that both American and Canadian states are listed on the check out page by default. If you make the United States the only country available in the country list, the Canadian states will still be listed with the American states in the state drop-down box. To change this, go through an order and view the page source when you get to the checkout page. You will see that the HTML code is now there. You can copy the code and paste it into the CheckOut.html template. Then you can delete any states that you do not want to appear. You can delete countries out of the list using this method as well.
